Their own silverware — since (at least in my area) most restaurants do not include a teaspoon as part of the normal place setting
A sharp steak knife
Their own bottled water
A toothpick
A toothbrush (dentists recommend one should always brush or floss after every meal)

We go to a restaurant that only has a Balsamic Vinaigrette dressing.... I hate Balsamic Vinegar!
Although tempted, I don't bring my own dressing.

Many years ago we were ordering dinner in a "better" restaurant in Sydney, Australia and asked if our meals came with salads..... the waitress said they could accommodate us and brought us small plates with some cut up iceburg lettuce. "Do you have any dressing?" we asked. "Sure" she said and promptly brought us a jar of mayonnaise with a spoon in it.
