■ Taking your order and bringing your food, then making themselves scarce until it's time to bring you the bill.
■ Introducing themselves by name ("HI!  I'm Murgatroid, and I'll be your server tonight!") — I assume that would irk off a lot of people (because they won't remember or don't care anyway; to them, the server is just another piece of restaurant equipment like the plates or the flatware) — although personally I like that, so if my server does do what I first mentioned, I can flag down one of the others and ask for him/her by name.

And I also agree — doubled, and in spades! — with SeattleParrotHead's example.  Dammit, don't do that!  If I'm going to give you a 75-100% tip, I'll be sure to let you know; you won't have to go trolling for it like you're dredging for oysters.

Every now and then when a server does that to me, I'll tell them "Hi, I'm Larry. I'll be your customer tonight." They never seem to be amused.
