Forgot to answer this one before - I use Paint Shop Pro - does the stuff that Photoshop does for the average user and cost MUCH less. I just made a page with the ones I have on my machine at work that I have made (I only make the front booklet because I use them in the thin cases) - have more at home:

I just usually start with an image that is around 900x1800 pixels - make 1/2 of it (900x900) the front part and the other 1/2 the songs list. I just choose a font that fits right and scale images to fit as well. I then use CoverXP (a freeware program) to print them out. A few of the covers from Alpine are actually scans of the shirts that I bought at the show and then I touch them up.
