There is the Aventura Spa Palace in the Mayan Riviera. Not the best beach, but the resort makes up for it. Adluts only, 2 person jacuzzi in each room, and the include some sightseeing along with their all inclusive package. www.palaceresorts.com

---I know it is not an island, but from the Mayan Riviera you can easily go over to Cozumel by high speed ferry.
The Sandals resorts are really nice also, and if you are a certified scuba diver, they include a 2 tank dive each day (except the day you arrive, and the day you depart) As well as the Couples Resorts in Jamaica.
We did St Lucia last, and we had a great time. My husband did scuba diving around the base of the Pitons and he said it was great!! St Lucia is less "touristy" also.
