I'm going to say "offending a co-worker".
Now I realize that covers a lot of territory, but that's just why I think it's the most logical answer. Too much cologne/perfume/aftershave; one's language; one's personal hygiene or lack of same; one's manner of following (or not following) office protocol; inappropriate clothing; even something as trivial as too many bathroom breaks — could be anything that sets someone else off and has them complaining to HR or upper management. And since most employers hold all the cards and employees themselves have few or no rights or recourse, the potential for getting canned for any of these reasons – or for no reason whatsoever — is always there.
